Discharge ends the hospital stay, not the recovery. Many older adults leave hospital still needing nursing, physiotherapy or help with daily tasks. Transition care for the elderly fills that gap with a short, structured stay between hospital and home.
Home is not always ready for that stage. Stairs, bathrooms without grab rails and an empty house during the day all raise the risk of a fall or a missed dose. Even Healthcare reports that nearly 10% of hospitalised patients return to hospital within 30 days of surgery, and 60% of those readmissions are preventable.
This guide explains what transition care is, who needs it and how it differs from assisted living. It then sorts recovery facilities in seven Indian cities into three types and lists monthly costs for each.
What is transition care for the elderly?
Transition care is short-term, structured care for patients who no longer need acute hospital treatment but cannot yet manage safely at home. HCAH, one of India’s transition care providers, describes it as intensive therapy and nursing for exactly this group. You may also see it called transitional care, step-down care or post-hospital recovery care.
A doctor assesses the parent, sets recovery goals and reviews progress during the stay. According to The Week’s report on transitional care centres, patients typically stay for weeks, and sometimes for months. The parent then goes home or moves to another care setting.
What transition care includes
Services vary by centre. Providers such as HCAH, Athulya and Antara list these as core components:
- Round-the-clock nursing care
- Doctor supervision, with regular reviews of the recovery plan
- Physiotherapy, plus occupational or speech therapy where needed
- Medication management
- Wound care and monitoring
- Nutrition support and planned meals
- Psychological support
Who needs transition care
Transition care suits a parent who is medically stable but not yet able to recover safely at home. Common situations include:
- Recovery after a knee or hip replacement
- Stroke or paralysis rehabilitation
- Recovery after cardiac or thoracic surgery
- Recovery after an ICU stay
- A fracture after a fall
A parent whose only need is daily help with bathing, meals or medication does not need transition care. That need points to assisted living instead.
Hidden obstacles at home after discharge
A home that worked before the hospital stay may not work during recovery. Check these seven points before the discharge date:
- Stairs between the bedroom and the toilet
- A bathroom with no grab rails or non-slip flooring
- No one at home during the day
- No system for tracking medication doses
- No walker, commode or hospital bed where the doctor advises one
- No caregiver trained for wound care, transfers or oxygen support
- No plan for who to call if the parent falls or feels unwell
Plan for an emergency before discharge
Ask the hospital for these five items before the parent leaves:
- A written discharge summary and a current medication list
- A list of warning signs that need a hospital visit
- The therapy plan, including how often physiotherapy is needed and for how long
- A list of equipment needed at home
- Dates for follow-up appointments
Keep the discharge summary, the doctor’s number and the name of the nearest hospital in one place. Share the same details with every person who will be at home with the parent.
Post hospital care for the elderly: three types of facility
Three types of facility offer recovery care in India. They differ in how much therapy they run and whether a parent can stay after recovery.
Type 1: Transition care and rehabilitation centres
These centres exist to get a parent through recovery. Care runs on a therapy plan with daily physiotherapy, nursing and doctor oversight. The stay ends when recovery goals are met, and the parent returns home or moves to another setting.
Type 2: Transition care with assisted living
These operators run recovery care and assisted living under one brand or one roof. A parent can recover first, then stay on if daily help is still needed. Many offer short stays priced by the month.
Type 3: Assisted-living-led homes
These homes focus on daily living support: meals, personal care, medication help and nursing oversight. Daily therapy is not their main service. Some offer short or trial stays.
The line between the types is blurry. An assisted-living-led home may offer physiotherapy, and a rehabilitation centre may offer long stays. Two questions sort most cases:
- Does the centre run a daily therapy plan?
- Can my parent stay after recovery ends?
Which type fits your parent
| If your parent | Type to consider |
|---|---|
| Needs daily therapy after a stroke, joint replacement, cardiac or thoracic surgery or ICU stay | Type 1 or Type 2 |
| Needs recovery care now and may need daily help for longer | Type 2 |
| Is medically stable but cannot manage bathing, meals or medication alone | Type 3 |
| Has dementia, or shows new confusion after discharge | Ask the treating doctor first, then consider memory care |
| Has family at home all day and can arrange a visiting nurse and physiotherapist | Home recovery care may be enough |

What happens after the recovery stay: assisted living after a hospital stay
A recovery stay ends with a decision. Five paths are common:
- Return home with a home caregiver or part-time support
- Stay on at the same operator under assisted living (Type 2)
- Move to an assisted-living-led home (Type 3)
- Move to memory care if cognitive decline becomes the main concern
- Take a short respite or trial stay before deciding
The treating team’s final assessment guides the choice. The ADL and assisted living guide explains how daily-living assessments work and when help becomes a daily need.
How to choose a transition care facility
Ask every facility these nine questions:
- Does the centre admit directly from hospital with a discharge summary?
- Which therapists are on the team, and how many sessions will my parent get each week?
- Is a doctor on site or on daily rounds, and what is the nurse-to-patient ratio?
- What is the usual recovery stay, and what does it cost?
- Can my parent stay after recovery? Do the room or the price change?
- What happens in a medical emergency, and which hospital is nearest?
- Which conditions does the centre not admit?
- What does the monthly fee cover, and what is billed separately, such as petty cash, attendants and medicines?
- How does the team report progress to the family, and who answers questions?
If your parent may stay beyond recovery, read our guide on how to choose assisted living in India. It covers the questions to ask about long-term fees, daily care and what a monthly rent includes.
FAQs
What is transition care for the elderly?
Transition care is short-term, structured recovery care for older adults who no longer need acute hospital treatment but cannot yet manage safely at home. It combines nursing, doctor supervision and therapy. It ends when recovery goals are met.
How long does transition care last?
Stays usually run from a few weeks to a few months. The treating team sets the length based on recovery progress. Sukino, for example, cites 6 to 8 weeks of multi-modal rehabilitation for a typical patient, according to BioSpectrum.
Can my parent stay on after transition care?
At Type 2 centres, yes. Operators such as Athulya, Antara and Geri Care run recovery stays and long stays. Ask whether the room, the care team and the price change when the stay type changes.
How much does transition care cost in India?
According to our research, monthly fees run from ₹30,000 for assisted-living-led homes to ₹2,40,000 for centres with skilled nursing and rehabilitation. Cost depends on the city, the room type and the caregiver ratio. Deposits, petty cash and GST come on top at several centres.
The bottom line
Start with how much recovery help your parent needs, not with a facility name. A parent in active recovery needs a therapy-led centre. A parent who is stable but needs daily help needs an assisted-living-led home. A parent who needs both fits a Type 2 operator.
